2015 saw the highest rate of Immediate Jeopardies received in Wisconsin since 2008.  This increase caused concern among Stakeholders, Leaders, and the State of Wisconsin Department of Quality Assurance.  The State DQA brought these stakeholders together to discuss the root cause of the IJ citation increase. Upon review of SOD’s it was clear to all stakeholders that leadership and a lack of systems implementation process was at the root cause.

A Civil Money Penalty Grant was sought by the Wisconsin Director of Nursing Council – Education Forum, Inc. to bring education to address this gap in knowledge.

Learner Outcomes

As a result of this activity, the participant will be able to:

  • Report an improvement in identifying effective leadership traits required to implement and evaluate clinical systems.
  • Report knowledge gain in identifying relevant data sources to create evidence based clinical operating systems
  • Report knowledge gain in developing and implementing clinical operating systems that will improve compliance with F157 (F580) – medical provider notification of resident change in condition.

Who Should Attend

At a minimum, the Administrator AND Director of Nurses attend the day- long session. Other members of the interdisciplinary or nursing team will be welcome.

At the end of the session the attendees will be required to develop an action plan for process improvement. Part of the learning experience is to be able to effectively implement a change in a system which will impact the quality of care of the resident. The team will need to engage their care giving team in this implementation to assure success. The monitoring and evaluation of the effectiveness/success of the project will include the effects on the resident population. The success will be evaluated during each of the follow up contacts with redirection provided as necessary.

tina-circle-black-and-whiteTina Belognia RN, BSN has over 20+ years in the post-acute care market, Tina’s strengths include survey readiness and management, risk analysis, high-risk medical record review, infection control, and program development and implementation. In past roles, Tina was responsible for the development and implementation of a large corporate-wide QAPI program, as well as a falls management program and a palliative care program. She has also served as a Senior Examiner with the AHCA Quality Award Program and has assisted numerous facilities in the Quality Award Application process. Tina is an active member of the American Association of Legal Nurse Consultants, an associate member of WHCA, and a business member of the WI Director of Nursing Council.

Lorilori-black-and-white-circle Koepell BSN, NHA       Lori has over 20 years of experience in the post-acute care setting including skilled nursing, pharmacy, and assisted living operations. Accomplishments include multi-facility clinical management of 7 skilled nursing facilities in Wisconsin and Missouri. Turned around operation of a government ran, 320 bed skilled nursing facility. Turn around project resulted in reduction of annual tax levy from $5.5 to $1.6 million dollars. The turnaround facility received recognition as one of the top 10% nursing homes in the nation in 2012 and 2013, Women’s Choice Award for 2013, and the Wisconsin Forward award. Koeppel received the Facility Leadership Award from the American College of Health Care Administrators in 2012.


Jojoey-black-and-white-circleey Pettis RN, DNS-CT, WCC, DWC is Executive Director of the Wisconsin Director of Nursing Council – Education Forum, Inc.  In this role Joey has been invited to sit on many  improvement committees, including WHCA Quality Improvement Committee, Wisconsin Center for Nursing, and the WI HAI in LTC Prevention Coalition.  Over her career she had lead large and small skilled nursing facilities.  Joey’s strengths include leadership, systems implementation, process improvements, and wound care team management with a proven ability to mentor and engage team with proven results.

Education Credits

This activity has been submitted to the Wisconsin Nurses Association Continuing Education Approval Program Committee for approval to award six contact hours. Wisconsin Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.  WNA requires us to publish the following statement:

“Approval of this activity does not imply endorsement by WNA, CEAP or ANCC of any commercial products displayed in conjunction with the educational activity.”

This program has been submitted (but not yet approved) for six continuing education clock hours from NAB/NCERS.

Each attendee will be required to turn in a completed evaluation form and a completed and signed copy of the “Approved Contact Hours Certificate.” You are asked to complete the evaluation after each speaker; review the objectives and document if the objectives were met for that specific session; then check on the Certificate form that you were present for the entire time to earn contact hours. The proper forms will be maintained by the Wisconsin Director of Nursing Council for the NAB, WNA, CEAP and various State Nursing Boards to confirm your participation.


In the event of adverse weather that would make it dangerous for the presenters and attendees to attend an email sent to the registered attendees at 6pm the day prior to the event.
